Official Practice Dates for the 2011-2012 Sports Seasons
Fall – August 15  ~  Winter – November 18  ~  Spring – March 5

PIAA HEALTH REGULATIONS MANDATE
Prior to any student participating in Practices, Inter-School Practices, Scrimmages
and/or Contests, at any PIAA member school, the student is required to complete
a physical evaluation. Physicals for the fall sports season need to be dated after
June 1, 2011. The PIAA form is available on this web site.

THIS IS THE ONLY FORM THAT WILL BE ACCEPTED!

REMINDER:
SECTION 4 MUST BE SIGNED, DATED AND CHECKED "CLEARED"
BY THE DOCTOR !

This form is for students who have already turned in a completed
PIAA physical form and are now going to participate
in a second or third sport this school year.

Click here for the Section 6 Certification by Licensed Physician of Medicine.
This form is for students who required medical treatment by a
physican since turning in a completed PIAA physical form.
